People with age-related macular degeneration may experience different
levels of concern and anxiety after diagnosis or may be grappling with
lifestyle adjustments as the disease progresses. The Duke Center for
Macular Diseases has a patient care manager who is available to assist
patients and their loved ones in coping with the challenges associated
with macular degeneration. The patient care manager provides the
following services:
Consultation for patients, families and/or caregivers:

In the patient care manager's office

By a telephone appointment

Through e-mail
Individualized, comprehensive care planning:

Assessments of the patient and his/her their family's emotional,
social and environmental issues related to age-related macular
degeneration
Support program:

Telephone patient-to-patient support program

Support groups for patients and caregivers
Resource and referral to community resources
